It is our policy to welcome views from patients, residents, visitors and the community about the services we provide. Any complaint will be viewed as an opportunity to learn lessons and improve services. All complaints will be taken seriously, handled appropriately and sensitively.
If you wish to make a complaint please print and complete the attached form and post to our Complaints Officer as outlined on the form.
A copy of our Complaints Policy is available from the Complaints Officer if required.
